Carpal tunnel syndrome is the most common nerve compression condition, affecting approximately 3–6% of the population. What Is Carpal Tunnel Syndrome?
The carpal tunnel is a narrow passageway in the wrist formed by the carpal bones and the transverse carpal ligament. The median nerve and nine tendons pass through this tunnel. When the tunnel becomes narrowed — due to swelling, repetitive strain, or structural changes — the median nerve is compressed, causing the characteristic symptoms.
Classic Symptoms

- Ergonomic assessment: Identifying and modifying the workplace and home activities that are loading the wrist repetitively
- Cervical and thoracic assessment: The median nerve originates from the cervical spine; neck stiffness and thoracic outlet issues can contribute to or mimic CTS symptoms
When Is Surgery Needed?
Surgical carpal tunnel release is very effective for severe or refractory cases. It is typically recommended when: symptoms are constant and severe; there is significant thenar muscle wasting; nerve conduction studies confirm severe compression; or conservative treatment has failed after 3–6 months. Your physiotherapist will refer for surgical review if you are not progressing adequately.
