Hip pain can significantly affect your ability to walk, climb stairs, sleep comfortably, and remain active. Whether your hip pain is in the groin, deep in the joint, or on the outer side, physiotherapy is an effective and evidence-based treatment for most hip conditions — often without the need for injections or surgery.

Hip Conditions We Treat

Hip Osteoarthritis

Hip osteoarthritis is one of the most common causes of hip pain in adults over 50. It causes groin and anterior hip pain, stiffness (especially in the morning), and pain with walking, stairs, and hip rotation. Contrary to common belief, exercise is the most effective treatment for hip OA — not rest. Targeted physiotherapy strengthens the muscles that support the hip joint, reducing load on the cartilage and significantly improving pain and function.

Gluteal Tendinopathy

Gluteal tendinopathy — previously called trochanteric bursitis — causes pain on the outer side of the hip, often radiating down the outer thigh. It is particularly common in women over 40 and worsens with walking, stairs, crossing the legs, and sleeping on the affected side. The key to treatment is a specific, progressive tendon loading program — not rest, stretching, or injections alone.

Hip Replacement Rehabilitation

Total hip replacement (THR) is one of the most successful operations in medicine — but the outcome depends significantly on the quality of rehabilitation. Physiotherapy after hip replacement focuses on restoring safe movement within surgical precautions, rebuilding hip and gluteal strength, and returning to walking, stairs, and daily activities. Most patients need 3–6 months of physiotherapy for a full recovery.

Femoroacetabular Impingement (FAI)

FAI occurs when the ball and socket of the hip joint do not fit together perfectly, causing pinching with hip flexion and rotation. It is common in young active adults and athletes. Physiotherapy addresses the movement patterns and muscle imbalances that aggravate impingement, and can be highly effective in managing symptoms without surgery.

Hip Flexor Strains and Tendinopathy

Hip flexor injuries are common in runners, cyclists, and football players. They cause pain in the front of the hip and groin, worsened by lifting the knee and hip flexion against resistance. Physiotherapy uses progressive loading and biomechanical correction to rehabilitate the tendon and prevent recurrence.

Piriformis Syndrome

The piriformis muscle deep in the buttock can tighten and compress the sciatic nerve, causing buttock pain and sciatica-like symptoms. Treatment includes soft tissue therapy, dry needling, and targeted hip strengthening.

What to Expect at Your First Appointment

Hip pain assessment at Better Physiotherapy Bundoora includes a detailed movement screen, hip joint testing, muscle strength assessment, and biomechanical analysis of your gait. Treatment begins in the same session. Your physiotherapist will give you a clear diagnosis, treatment plan, and realistic timeline at the end of your first appointment.

Frequently Asked Questions

My hip pain is on the side, not in the groin — what is it?

Lateral hip pain is most commonly gluteal tendinopathy or hip bursitis. It is often worse with walking, stairs, and sleeping on the affected side. This presentation responds very well to a specific physiotherapy loading program — complete rest actually makes it worse.

Can physio delay or prevent hip replacement?

Frequently yes. Exercise therapy is the number one evidence-based treatment for hip OA. Many patients with moderate to severe arthritis find they can significantly reduce pain and improve function with a structured physio program, delaying or avoiding surgery.

How soon after hip replacement surgery should I see a physio?

Ideally within days — many hospitals begin physiotherapy on the day of or day after surgery. Once discharged, outpatient physiotherapy should begin within 1–2 weeks. Early, progressive rehabilitation produces significantly better final outcomes.

What exercises are bad for hip osteoarthritis?

No exercise is universally "bad" for hip OA — it depends on your specific presentation. High-impact activities like running may need to be modified temporarily, but progressive strengthening exercise is actually protective. A physiotherapist will design an individualised program.
