Hydrotherapy — also called aquatic physiotherapy — uses the unique properties of water to enable exercise that would be impossible or extremely painful on land. When joint pain, neurological conditions, or post-surgical restrictions make standard physiotherapy too difficult, hydrotherapy provides a pathway to movement, strength, and recovery.

Why Water Works

Water has four therapeutic properties that make it uniquely suited for rehabilitation:

  • Buoyancy: Reduces effective body weight by up to 90% when submerged to the neck, dramatically reducing joint load and allowing movement that would be impossible on land
  • Hydrostatic pressure: The pressure of water reduces swelling, improves circulation, and provides proprioceptive feedback to joints
  • Viscosity: Water provides resistance in all directions simultaneously, building strength through full range of motion without heavy weights
  • Warmth: Heated pool water (33–35°C) reduces muscle tension, improves flexibility, and decreases pain

Conditions Best Suited to Hydrotherapy

  • Severe knee or hip osteoarthritis: When walking or land-based exercise is too painful, the pool allows movement and strengthening
  • Early post-surgical rehabilitation: After joint replacement or spinal surgery, hydrotherapy allows cardiovascular exercise and gentle strengthening while protecting the healing tissue
  • Neurological conditions: Stroke, MS, Parkinson's — the water's support allows safe practice of movement and balance
  • Severe chronic pain and fibromyalgia: The warm, supportive environment allows exercise for patients who find land-based exercise intolerable
  • Rheumatoid arthritis: During flares, warm water hydrotherapy maintains movement without stressing inflamed joints
  • Paediatric conditions: Children with developmental conditions, cerebral palsy, or congenital musculoskeletal issues

Hydrotherapy vs Land-Based Physiotherapy

For most musculoskeletal conditions, land-based exercise is more effective than hydrotherapy because it provides higher-load strengthening and more functional movement patterns. Hydrotherapy is not a superior form of physiotherapy — it is the right choice for specific patient groups who cannot yet tolerate land-based exercise, or as a bridge to progressively higher land-based loads.

What to Expect in a Hydrotherapy Session

Sessions are typically 30–45 minutes in a heated therapeutic pool under direct supervision of a physiotherapist. Exercises are individually prescribed for your condition and goals, and progress session by session. Most patients transition to land-based exercise as their strength and tolerance improve.

Frequently Asked Questions

Do I need to be able to swim for hydrotherapy?

No. Hydrotherapy pools are shallow (typically chest height) and the exercises are performed standing or with pool floatation aids. Swimming ability is not required.

How warm is a hydrotherapy pool?

Therapeutic hydrotherapy pools are heated to 33–35°C — significantly warmer than a standard swimming pool. The warmth helps relax muscles and reduce pain, making exercise easier and more comfortable.

Is hydrotherapy the same as swimming?

No. Hydrotherapy is specific, physiotherapist-prescribed therapeutic exercise in a heated pool. It is not recreational swimming or aqua aerobics — the exercises are individually designed for your condition and goals.
