Sustained postures — regardless of whether they are "good" or "bad" — produce tissue loading that accumulates over time. The primary drivers of postural pain are:

  • Muscle fatigue and imbalance: Prolonged desk work overactivates upper trapezius and pectorals, while underusing deep neck flexors, lower trapezius, and serratus anterior — creating a predictable pattern of muscle weakness and tightness
  • Insufficient movement: The human body is designed for movement, not static loading. Regular position changes reduce cumulative tissue stress more than any single "correct" posture
  • Thoracic stiffness: Reduced thoracic extension increases cervical and lumbar compensatory loading

Forward head posture (FHP) — where the head sits forward of the shoulders — is extremely common in desk workers and screen users. For every centimetre the head moves forward, the effective load on the cervical spine increases. FHP contributes to upper trapezius overactivation, deep neck flexor weakness, and cervicogenic headaches. Physiotherapy addresses both the symptoms and the underlying muscle imbalance.

What exercises help with poor posture?

The most evidence-based exercises for postural pain are: chin retractions (deep neck flexor activation), lower trapezius exercises (Y/T/W raises in prone), scapular retractions, thoracic extension over a foam roller, and hip flexor stretching. However, the specific program should be based on individual assessment — different people have different postural deficits.

Are posture braces helpful?
